EnFlux’s team was mandated to perform a field inspection and determine the root cause of failure. After completing the preliminary inspection, the EnFlux team was assigned to review all the available documentation, provide a scope for further investigation to verify and validate the root cause of failure, as well as lead and complete a Root Cause Analysis. EnFlux staff collaborated with other consulting engineers and produced a Root Cause Analysis report for the penstock failure.
